- CLINIQON IS LOOKING FOR HOME HEALTH CASE MANAGEMENT NURSES! - Duties \& Responsibilities: - Obtain and follow up on Face-to-Face (F2F) encounter documentation from physicians - Ensure timeliness, accuracy, and completeness of all home health required documents - Review and monitor patient charts for eligibility, physician orders, recertifications, and plan of care compliance - Coordinate with physicians, clinicians, and internal departments for missing or pending requirements - Track and escalate any delays, missing signatures, expiring orders, or compliance risks - Collaborate with intake/admissions team to support case initiation readiness - Maintain organized documentation logs and update EMR accurately - Exercise critical thinking to prevent denials, delays, or compliance issues - Ensure all communication and follow-ups are professional, proactive, and documented - Given the compliance-sensitive nature of this role, we require individuals with prior Home Health experience, documentation skills, and excellent follow-through.
