We specialize in delivering personalized care through Nurse Practitioners and other healthcare providers in the comfort of patients' homes. We are seeking a highly organized and compassionate Scheduler & Patient Coordinator to join our growing team. Responsibilities Schedule appointments for patient visits in alignment with the provider’s availability and patient needs. Coordinate with healthcare providers to ensure timely and efficient patient care. Manage patient intake, ensuring accurate patient information is captured and processed. Maintain communication with patients regarding appointment reminders and any necessary pre-visit instructions. Ensure accurate and timely documentation in our electronic health records (EHR) system. Act as a liaison between patients and the medical team to ensure optimal patient care. Assist with patient follow-up and track ongoing care needs. Qualifications Experience in scheduling, patient coordination, or customer service in a healthcare setting preferred. Recent graduates are encouraged to apply. Strong organizational and multitasking abilities. Proficiency in Microsoft Office and familiarity with EHR systems. Excellent communication skills and attention to detail.
