We're looking for a detail-oriented Admissions Support to join our remote team. In this role, you'll play a key part in managing patient medical records, preparing authorization requests, processing essential documents, coordinating with hospitals and vendors, and ensuring the accuracy and efficiency of admissions operations. Key Responsibilities File Management of Patient’s Medical Records Manage incoming emails from admissions. OCR scan and file SFMC and RCH daily/weekend assignments. Download and OCR scan patient’s files. Upload and file patient’s medical records to the centralized storage of medical records. Document Processing Data entry of patient’s medical records. Create and attach BH cover sheets. Bookmark and highlight 6 points of care. Auth Request Preparation Generate service costs for the required plan of care using the Service Calculator. Collaborate with the Admin Support of Finance & Operations Department for LOA preparation. Track financial approval status. LOA Processing Make patient calls for discharge confirmation. Draft and prepare quotes for vendor agreements using DocuSign. Send vendor sign agreements via DocuSign. Track vendor sign status. Hospital Calls Confirm if patients are still admitted and update room numbers. Check with admitting for discharge dates and verify discharges from the previous day. Log Updates Update room numbers, discharge locations, and notes in the log sheet. Update vendor tracking information. File Management Send PDFs to James with active cases and discharged lists. File active cases. Admissions Coordination & Patient Care Support Receive and process inbound referrals from hospitals, discharge planners, social workers, and case managers for skilled nursing and subacute care. Review patient clinical documents including face sheets, History and Physical reports, medication lists, and physician orders to assess admission eligibility. Coordinate with facility admissions teams, Directors of Nursing, and administrator contacts to confirm bed availability and obtain admission approvals. Track referral-to-admission timelines and ensure all admission activities are documented accurately in real time. Communicate with patients’ families and responsible parties to explain care options, timelines, and next steps. Liaise with managed care health plan case managers, Medi-Cal representatives, and managed care coordinators to facilitate authorization and coverage verification. Assist in obtaining prior authorizations and verifying insurance eligibility for prospective admissions. Support the preparation and submission of Level of Care assessments and preadmission paperwork in compliance with Title 22 and state regulations. Maintain accurate referral logs, admission tracking sheets, and outcome reports using shared platforms and internal tools. Escalate complex or time-sensitive cases to the Principal Resource Director and Admissions Lead and ensure that discharge deadlines are met with appropriate urgency. Follow up on pending referrals and maintain consistent communication with referral sources to protect and strengthen relationships. Qualifications Bachelor’s degree in Healthcare Administration, Business Administration, or a related field. Proven experience in admissions or medical records role. Strong attention to detail and organizational skills. Excellent communication and interpersonal skills. Proficiency in medical record management software and Microsoft Office Suite. Skills Analytical thinking and problem-solving. Ability to work independently and as part of a team. Time management and ability to meet deadlines. Strong ethical standards and integrity. Organizational Skills: Ability to manage multiple priorities, maintain records, and ensure smooth operations. Technical Proficiency: Competence in using shared trackers, checklists, and project management tools. Admissions Knowledge: Demonstrated experience or strong understanding of skilled nursing and post-acute admissions, post-acute care coordination, and Medi-Cal/managed care coordination. Initiative: Ability to take ownership of special projects and drive continuous improvement in administrative and admissions functions. Advanced proficiency in Microsoft Excel, including pivot tables, macros, and complex formulas (VLOOKUP, XLOOKUP, MATCH, INDEX, etc.). Flexibility to adapt to shifting priorities and tasks.
