We are seeking a detail-oriented and experienced Home Health Medical Coder to join our team. In this role, you will play a critical part in ensuring accurate coding and compliance with healthcare regulations. If you have strong analytical skills, a deep understanding of coding systems, and experience in home health, we encourage you to apply. Responsibilities Analyze patients’ charts thoroughly to ensure accurate coding Utilize specialized medical classification software to assign diagnosis codes Assign codes to diagnoses using ICD-10-CM codes in accordance with PDGM guidelines by CMS Enter coding information into an online system Ensure codes are accurate and properly sequenced based on government and insurance regulations Follow up with providers regarding incomplete or unclear documentation Communicate effectively with clinical staff about documentation requirements Research complex or unusual coding cases as needed Stay updated on changes in coding rules and regulations Manage detailed and highly specific coded data Maintain strict patient confidentiality and data security Ensure all codes used are current and valid Requirements Minimum of 2 years experience in Home Health coding Experience with Plan of Care (POC) and OASIS Review Certified Professional Coder (CPC) certification required HCS-O or HCS-D certification is a plus Strong knowledge of ICD-10-CM, CPT, and HCPCS coding systems Excellent communication and collaboration skills Ability to work independently and within a team Must be available to start JULY 2026 Why Cliniqon?
