As a Home Health Clinical Intake Coordinator , you'll play a critical role in ensuring a seamless transition of patients into home health and hospice services. What You'll Do Clinical Intake & Referral Management Review referrals and clinical documentation to determine eligibility for home health and hospice services Ensure medical necessity is supported by clinical records and payer guidelines Evaluate documentation for completeness before Start of Care (SOC) Insurance Verification & Authorization Verify patient insurance coverage and eligibility Coordinate with authorization teams to obtain required approvals Navigate insurance portals to confirm benefits and authorization status Patient Records & Documentation Create and maintain accurate patient records in MatrixCare EMR. Gather and organize: Discharge summaries Face-to-face encounter documentation Physician consult notes Patient demographics Insurance information Medical history Facesheets and other required clinical documents Follow up with providers to obtain missing or incomplete documentation Physician & Clinical Coordination Confirm physician orders and ongoing physician involvement Collaborate with RN Supervisors, Case Managers, Clinicians, Social Workers, Medical Assistants, Marketers, and Scheduling Teams Conduct patient background checks to help ensure clinician safety Patient & Family Support Conduct welcome calls with patients and families Answer questions regarding the admission process and Start of Care Provide compassionate guidance throughout the onboarding experience Stakeholder Collaboration Coordinate with business development teams and referral partners to facilitate smooth patient admissions Route incoming calls to the appropriate departments Communicate hospitalization updates and transitions of care with all involved stakeholders Hospitalization Monitoring Track patient hospitalizations Keep clinical teams informed of status changes and care transitions Systems & Portal Management Work confidently within multiple healthcare platforms, including: MatrixCare EMR Epic CarePort Allscripts CarePort Navi Ensocare Availity UHC Portal NGSConnex CT DSS PointClickCare Compliance & Quality Ensure compliance with CMS, state, payer, and agency requirements. Maintain strict HIPAA compliance in all communications and documentation. Identify opportunities to improve intake workflows and efficiency. Team Support Cross-cover fellow Intake Coordinators during high-volume periods. Assist with both Home Health and Hospice admissions. Support onboarding and training of new intake team members as needed.
