We are hiring!! QA/Patient Care Coordinator for Home Health Qualifications Registered Nurse (RN) Home Health experience is a must . Hands-on experience using WellSky/Kinnser EMR . Strong understanding of OASIS documentation , Medicare home health regulations, and Plan of Care (POC) review. QA nursing documentation for accuracy and compliance. Prepare prior authorization requests and follow up with insurance companies. Communicate with PCP offices as needed. Perform data entry, including maintaining medication lists and updating care plans. Schedule RN visits for our team of nurses. Track outstanding clinical documentation and follow up as needed. Support compliance and quality assurance activities. Perform other administrative and clinical support duties as assigned..
