Job Summary We are seeking a detail-oriented, qualified Care Coordinator to support our US-based Chronic Care Management (CCM) program. In this role, you will work under the general supervision of a licensed US physician to help patients manage chronic conditions and navigate the healthcare system. This remote position focuses heavily on care coordination and administrative workflows, but includes structured, low-risk clinical coordination tasks. The ideal candidate maintains strict compliance with HIPAA privacy standards, understands US healthcare systems, and holds the necessary US medical licensure or certification required to legally log clinical care minutes under Medicare guidelines. Key Responsibilities Patient Coordination & Communication (Clinical & Administrative) Patient Outreach: Conduct regular telephonic touchpoints with patients to track health goals, review care plans, and monitor compliance. Symptom Tracker: Gather routine health updates and identify potential changes in patient health status to flag for the supervising physician. Medication Tracking: Review medication lists with patients, confirm adherence, and facilitate prescription refill requests. Care Navigation: Assist patients with appointment scheduling, specialist referrals, and coordinating community resources. Family & Care Team Liaison: Act as the central point of contact to update authorized family members, designated caregivers, and the patient's multidisciplinary care team (e.g., specialists, physical therapists, home health agencies) regarding changes in the care plan, upcoming appointments, and physician instructions. EHR Documentation & Compliance CCM Billing Logs: Document all patient interaction time accurately within the Electronic Health Record (EHR) to satisfy CMS billing requirements for clinical staff minutes. Data Compilation: Collect health data, lab results, and discharge summaries from external healthcare entities to maintain a comprehensive digital care profile. Data Security: Strict adherence to HIPAA regulations regarding the management of Protected Health Information (PHI). Required Licenses & Certifications Candidates must hold at least one of the following active credentials valid in the United States: State Nursing License: Active Registered Nurse (RN) or Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) license. National Medical Assistant Certification: Certified Medical Assistant (CMA from AAMA) or Registered Medical Assistant (RMA from AMT), provided the candidate's state medical board allows MAs to execute phone care coordination under general physician supervision. Preferred (Optional) Certifications Certified Case Manager (CCM) or Chronic Care Professional (CCP) credentials. Qualifications & Skills Experience & Education Education: Associate's or Bachelor's degree in Nursing, Healthcare Administration, or an allied health field. Experience: Minimum of 1–2 years of experience in US healthcare settings, primary care clinics, or digital care management programs. Core Competencies US Healthcare Knowledge: Understanding of CMS CCM billing codes, Medicare regulations, and preventive healthcare. Communication: Exceptional verbal and written English; ability to communicate empathetically with elderly or chronically ill demographics. Technical Savviness: High proficiency with telehealth and EHR platforms Position Details Supervision: Reports directly to the Clinical Lead with general supervision by a licensed US Physician.
