Registered Nurse (RN) Intake Coordinator – Home Health Remote | Full-Time Home Health Experience is REQUIRED Are you an experienced Registered Nurse (RN) with a strong background in Home Health Intake ? Do you thrive in a fast-paced environment where your clinical expertise and coordination skills directly impact patient care? We are seeking a highly organized and detail-oriented RN Intake Coordinator to join our growing team. This role is ideal for an RN who understands the home health admission process from referral through Start of Care (SOC) and is passionate about delivering exceptional patient experiences. What You'll Do? As our RN Intake Coordinator, you will play a critical role in ensuring timely, accurate, and compliant patient admissions. Clinical & Non-Clinical Intake Review referrals and clinical documentation to determine eligibility for Home Health and Hospice services Ensure medical necessity is supported by physician documentation and payer guidelines Coordinate Start of Care (SOC) admissions from referral through onboarding Insurance Verification & Authorizations Verify insurance eligibility and benefits Coordinate prior authorizations with insurance and authorization teams. Resolve coverage issues to prevent delays in patient care Documentation & EMR Management Create and maintain patient records in MatrixCare Collect and organize: Face-to-Face documentation Physician orders Discharge summaries Consult notes Facesheets Insurance information Demographics Medical history Ensure documentation is complete before SOC Clinical Coordination Confirm physician orders and physician involvement Collaborate with RN Supervisors, Case Managers, Clinicians, Medical Assistants, Marketers, and Social Workers Conduct patient background reviews to promote clinician safety Patient & Family Support Conduct welcome calls with patients and families Educate patients regarding the admission process and Start of Care Address questions related to Home Health admissions Referral & Stakeholder Management Coordinate with hospitals, physicians, referral sources, business development teams, scheduling staff, and other stakeholders Route incoming referrals and admission inquiries appropriately Provide updates regarding hospitalization status and transitions of care Hospitalization Monitoring Monitor patient hospitalizations Communicate updates promptly to the care team to ensure continuity of care Portal & System Navigation Experience using or familiarity with: MatrixCare Epic CarePort Allscripts CarePort Navi Ensocare Availity UHC Portal NGSConnex CT DSS PointClickCare Communication Manage referral emails and fax communications Follow up with providers for missing documentation Ensure timely communication with referral partners and clinical teams Compliance & Quality Maintain compliance with CMS, state, payer, and HIPAA regulations Support quality assurance initiatives and process improvements Team Support Assist with onboarding and training new intake coordinators when needed Provide cross-coverage during high-volume periods Support both Home Health and Hospice admissions Qualifications Active Registered Nurse (RN) license Minimum of 1 year of RN experience in a hospital setting Home Health Intake experience is REQUIRED Strong understanding of Home Health workflows, referrals, admissions, and Start of Care processes Knowledge of medical documentation, insurance verification, and referral management Experience using EMR systems and referral/insurance portals Exceptional organizational, communication, and multitasking skills Ability to work independently while collaborating effectively with multidisciplinary teams Preferred Home Health and Hospice intake coordination experience MatrixCare experience Experience working with CMS regulations and Medicare Home Health guidelines Why Join Us?
