Intake Coordinator Bilingual (Spanish/English) Role / Job Description: The Intake Coordinator is responsible for managing the admission process for new patients receiving home health services. This role requires excellent organizational and communication skills, strong attention to detail, and a solid understanding of home health care procedures, insurance verification, and regulatory requirements. Key Responsibilities Review and evaluate patient referrals to determine eligibility for home health services. Coordinate and schedule initial assessments and admissions with healthcare professionals. Collect, verify, and maintain accurate patient information, insurance details, and medical records. Ensure all intake documentation is complete, accurate, and compliant with agency and regulatory requirements. Serve as a liaison between patients, families, physicians, healthcare providers, and insurance companies. Address and resolve issues related to the intake process, including insurance authorizations, referral requirements, and scheduling conflicts. Maintain timely communication with referral sources and internal departments to facilitate smooth patient admissions. Ensure compliance with Medicare, Medicaid, and other payer guidelines, as applicable. Perform other duties related to intake and patient coordination as assigned. Qualifications Minimum of 2 years of Home Health or Home Care experience . Knowledge of Home Health authorizations and insurance verification processes. Strong communication, interpersonal, and organizational skills. Detail-oriented with the ability to manage multiple tasks and priorities. Professional, compassionate, and team-oriented. Commitment to providing outstanding service to patients and their families. Preferred Qualifications Bilingual (Spanish/English) communication skills are an advantage.
