Cliniqon is hiring HOME HEALTH CLINICAL INTAKE COORDINATORS Key Responsibilities Patient Information Collection Demographic Data : Gather personal details such as name, address, date of birth, contact information, insurance details, and emergency contact. Medical History : Obtain past and current medical history, including diagnoses, medications, allergies, and surgeries. Current Condition : Document the patient’s present health status, functional limitations, cognitive abilities, and any acute or chronic conditions that require attention. Physician Orders and Referrals Physician Referral : Ensure the referral comes from a qualified physician who has reviewed the patient’s condition and deems home health care necessary. Review Physician Orders : Clarify and verify orders for nursing, therapy, or other services prescribed, such as physical therapy, occupational therapy, or speech therapy. Certification of Need : Confirm that the physician certifies the patient's need for home health services. Assessment of Care Needs Home Health Care Needs : Evaluate the patient’s needs for skilled nursing, physical therapy, occupational therapy, speech therapy, or home health aide services. Initial Assessment Coordination : Arrange for the initial visit to assess the patient’s home environment, potential risks, and the level of care required. Documentation and Compliance Complete Intake Forms : Fill out all required intake forms accurately and timely, ensuring compliance with regulatory requirements (e.g., Medicare/Medicaid guidelines). HIPAA Compliance : Ensure that all patient information is handled in accordance with HIPAA and other privacy laws. Document Care Orders : Properly document the patient’s orders, care plan, and ongoing care coordination within the electronic medical record (EMR) system.
